The forms below will be required to be downloaded and completed – where applicable – and uploaded when prompted on the SU Funding Opportunities for Undergraduates (SUFO) application form.
We recommend you complete the forms before you start filling out the application form and save these in a clearly marked folder, for example, SUFO application.

This form is to be signed by the student, the parent(s) or legal guardian(s)/guardian(s), and if applicable, the student’s spouse.
Please refer to the Types of Funding > SU External Donor Funding filter list to review the eligibility criteria for each bursary and see whether you qualify to apply (the Dippenaar, DW Ackermann, Piet Neethling, and Rodney Africa bursaries are faculty specific). Most of these forms are bilingual (ENG/AFR).
Please make sure you download the correct form: -CUG for current students; -PUG for prospective students. FORM SHEILA BRAND is only available for current students.
